At Bouchard Dental, your first appointment includes a thorough exam and gathering any information that our dentist must be aware of in order to provide care specifically to your oral health. We understand that all patients are different and require individualized care. 

Appointment Cancellation Policy

Kindly provide us with more than a 24-hour notice for any appointment that needs to be rescheduled. Appointments cancelled in less than 24 hours or missed appointment may be subject to a cancellation charge.

Financial Policy

We accept payment made with cash, check, debit cards, VISA/MC, Discover and American Express. We also offer 12 month interest-free financing through Care Credit. All payments are due at time of service.

Insurance Accepted - ask us for which providers we accept.

Your insurance policy is a contract between you and your insurance company. We accept most insurances, but it is your responsibility to know your insurance benefits. We will process all insurance claims as a courtesy to our patients. You will be provided with an estimated co-pay based on the information your insurance company has given us. The estimated co-pay is not a guarantee of benefits. Any portion not covered by dental insurance is the patient’s financial responsibility.
